To access this service an assessment by the Local Authority is required.
What
We provide support so young people aged 18 - 25 making the transition into adulthood are ready for the future and the move towards Adult Services, whether that’s with us or with another provider.
Providing support
Full support can be provided for all care needs, and all of our staff are highly trained and experienced in a variety of care techniques. Staff work closely with social workers and health care professionals to ensure all needs are met, as well as support to access the local community. Bespoke timetables based on each individual’s needs are created to include physiotherapy, life skills, sensory activities, social interactions, and much more, our support workers want to help everyone to live their lives as independently as possible.
Shapes Transitions
Transitions day care is provided at Lily’s Place and Lawford Lodge for children and young adults with profound physical and learning disabilities. Facilities also include fully equipped bathrooms, rooms with hoists and specialist equipment, a soft play area, sensory room, ball pool, and garden.
