This will be available initially through four primary care networks (PCNs) incorporating over 20 GP surgeries across Poole, Bournemouth Central and Purbeck. Referrals can be made via GP surgeries by parents/carers or the young person themselves. The sessions can take place in a school environment, at a GP surgery or online.


What

Active Monitoring gives people aged 11-18, through a programme of 1-2-1 guided self-help sessions, the opportunity to explore their feelings and emotions with a friendly trained practitioner.

Referrals can be made via GPs and healthcare professionals. This service is currently run out of certain GP surgeries. Please see our website for more information.

Active Monitoring provides pathways to help support mild to moderate mental health issues such as Grief and Loss, Anger, Low Mood, Self-esteem, and Anxiety and Stress. five half an hour sessions.

If your GP surgery is not listed that means we are not currently covering that area however please do keep checking as we are hoping to continue to expand; or take a look at our other services offered for children & young people.
